Hours of Work: Full Time 40 hours per week

Location: To work in the follow areas; Liverpool, Huyton, Prescot/Whiston/Rainhill, Halewood.

Job Purpose: To assess the Care needs of people living in their own homes and to work closely with both internal and external colleagues to provide a responsive and integrated Home Care Service, including specialist teams.

To directly line-manage Care/support workers.

To provide support to all Heron Care’s service user groups.

Lead on productivity within Locality.

Essential Qualifications: Qualifications are desirable but not essential. Experience working within care is required.

Essential Car User: Car driver is essential with access to own car.
